Please help me to complete this form to improve my overall appearnce of the wix website FEEDBACK FORM

I have created form for which netizens can help improve my website.The questions are mainly about the appearance of each page. There are 4 options per question and all question are mandatory. I have chose not to collect the users email and ask for personal information as they may feel uncomfortable to answer them and they may want to leave their identity anonymous. The last question is the opne question which allows respondents to give more comments on my website. I planned not to include this feedback form on my website as I am concerned about my identity as well. I do not wish any stranger to know my google account. My actual plan is to send this form to my friends and family and i hope that i can collect at least 10 feedback.
The photos below provide details how my form looks like and how they can fill in the form. Personally, I think that the form is useful as I will exactly know the area which I have to improve the website. Different people has different opinion so it would really help improve overall experience.
